At Signify Health, part of CVS Health®, we are seeking board-certified Physician Assistant/Physician Associate (PA) contractors to conduct in-home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members.

Job Summary

• Conduct in-home visits focusing on a person's overall health and well-being
• Identify potential barriers to care
• Earn up to $1800 to $3000 each week (Average 6-9 visits daily, $70 per completed assessment)
• Increase your per-visit rate up to 135% with our Market Multiplier*
• Earn additional compensation through our pay-per-test model
• Shifts available Monday - Sunday (no nights/weekends required)
• Short-Term or Long-Term basis
• Pre-scheduled in-home visits with health plan members
• Airfare, Hotel, and Rental Car available (depending on eligibility)
• Malpractice coverage
• Availability Incentives
• Independent Contractor, 1099 status
• Flexible Schedule
• State License Required (PA-C)

Responsibilities

• Conduct medication and medical history review
• Perform physical evaluations in a health plan member's home
• Populate patient-specific forms for each evaluation
