As a Physical Therapist at Harbor House Rehab & Nursing, you will be responsible for providing high-quality rehabilitative services to our residents. This includes assessing patients' conditions, developing personalized treatment plans, administering therapy, and monitoring progress. You will work closely with a multidisciplinary team to ensure the best possible outcomes for our residents.

Responsibilities

• Conduct comprehensive assessments of patients' conditions
• Develop individualized treatment plans based on patients' needs
• Implement therapeutic interventions to improve mobility, strength, and function
• Monitor and evaluate patients' progress and adjust treatment plans as needed
• Educate patients and their families on therapeutic exercises and techniques
• Collaborate with other healthcare professionals to provide coordinated care

Qualifications

• Active or pending Physical Therapy (PT) state license
• Proven track record of successful patient care experience
• Experience working with a geriatric disabled population preferred
