Job Schedule: Full Time Standard Hours: 40 Job Shift: Shift 1 Shift Details: 40 hours per week mix of day & evening shifts including rotating weekends & holidays

Job Description

As a Pharmacist at Hartford HealthCare, you will be responsible for performing functions related to the control, monitoring, and dispensing of medications for patients of all age groups. Your role will play a vital part in ensuring patient safety, optimizing drug therapy, and providing valuable drug information to the healthcare team. Responsibilities

• Supervise the work of supportive personnel to ensure safe and accurate preparation and dispensing of drugs
• Optimize drug therapy for the general patient population
• Provide direct patient care for pain management
• Maintain safety of drug therapy by identifying and resolving adverse drug events
• Provide drug information and facilitate effective patient care
• Investigate and resolve drug and pharmacy-related problems
• Document interventions, outcomes, and other statistics on a daily basis
• Adapt to meet age-specific patient needs
• May function as a charge pharmacist

Qualifications

• Pharm.D. degree required
• Current CT Pharmacist license or eligible within 45 days of hire
• PGY-1 Pharmacy residency preferred
• Prior experience with EPIC systems preferred
