Summary:
Hartford HealthCare is currently seeking a dedicated and experienced Clinical Team Manager (RN) to join our Homecare team. As the Clinical Team Manager, you will be responsible for managing daily operations of an interdisciplinary care team, ensuring the delivery of outcome-based, cost-effective care in a person-centered care model. Through your leadership, you will mentor and develop clinical staff, collaborate with team members, and strive for exceptional outcomes.

Responsibilities:
- Identify and facilitate professional development needs and competencies of staff through home visits to supervise and evaluate clinical performance
- Maintain compliance with regulatory policies and procedures by providing staff guidance in making clinical and case management decisions that are cost-effective and focused on clinical outcomes
- Accountable for team performance in achieving desired clinical and operational performance measures
- Utilize clinical software to identify opportunities for education, appropriate utilization of services, oversee clinical progression of patients, and trend team outcomes and performance
- Ensure clinical and functional outcomes are achieved at the highest level possible
- Resolve patient care problems collaboratively with internal and external customers
- Oversee daily assignments for clinical and clerical staff, ensuring team productivity standards
- Ensure timeliness of Start of Care (SOC) and adjunct service delivery
- Determine staffing needs and coordinate hiring to meet service demands
- Hire, train, coach, and evaluate staff according to agency timelines
- Review and analyze team and patient metrics consistent with agency initiatives
- Participate in preceptor programs
- Anticipate customer needs and respond quickly, accurately, and pleasantly
- Actively participate in performance improvement process and clinical huddles
Qualifications:
Nursing:
- Licensure: Registered Nurse with a license to practice in the State of Connecticut
- Education: According to current state requirements
- Experience: According to current state licensure regulations. OASIS and chronic care management certification required.
Therapy:
- Licensure: Physical Therapist, Occupational Therapist, or Speech Therapist with a license to practice in the State of Connecticut
- Education: Bachelor's degree in one of the therapy disciplines
- Experience: Minimum of 5 years clinical experience. Home health experience required and prior supervisory experience preferred. OASIS and case management certification.
